Confirmation Packet

The following paperwork is required in order for your son to attend Camp Wheeler. The paperwork must be completed and handed in during Orientation. Orientation is highly recommended.  If you cannot attend the orientation session, please send the completed paperwork with your son the first day of camp or he will miss valuable program time until we receive all the information.

1. Camp Wheeler Program Information - Contains important information about what your son needs to bring to camp, what to wear, camp times, orientation, the family campfire and camp absence.
2. Annual Health and Medical Form - All campers, staff, leaders and volunteers attending camp must completely fill out parts "A" and "C". This form must be completed annually and forms for those under 18 need to be signed by a parent/guardian. No doctor visit or signature is necessary. Be sure to keep the original for your records and make a copy for the camp. No one can attend camp without this form.
3. Emergency Contact Names - You must complete this form and bring it to camp orientation or the first day of camp
